Wellness Disclaimer, Liability Release, and Client Consent Agreement
Comfort Flow Recovery provides wellness, relaxation, mobility, circulation support, and recovery-focused services. Our services are not medical treatment and are not intended to diagnose, treat, cure, or prevent any disease, illness, or medical condition.
Comfort Flow Recovery services are not a substitute for medical evaluation, diagnosis, treatment, physical therapy, chiropractic care, or professional healthcare services. Clients should consult their physician regarding any medical concerns.
Clients agree to provide accurate and complete health information prior to receiving services. Failure to disclose medical conditions, medications, implants, surgeries, or other relevant health information may increase the risk of adverse effects.
Clients must inform Comfort Flow Recovery of any significant health conditions, including but not limited to:
Comfort Flow Recovery reserves the right to decline or modify services based on health and safety considerations.
Clients are responsible for immediately informing the provider if they experience discomfort, dizziness, pain, shortness of breath, unusual symptoms, or wish to discontinue any part of the session.
Individual results vary. Comfort Flow Recovery makes no guarantees regarding outcomes, benefits, improvements, or symptom relief.
Clients are responsible for providing a safe, clean, and suitable environment for mobile wellness services. Comfort Flow Recovery reserves the right to refuse service if conditions are unsafe.
Comfort Flow Recovery maintains a strictly professional wellness environment. Any inappropriate behavior, harassment, sexual comments, or requests for services outside the scope of professional wellness care will result in immediate termination of the session without refund.
To the fullest extent permitted by law, clients voluntarily assume responsibility for participation in Comfort Flow Recovery services and release Comfort Flow Recovery, its owners, representatives, and contractors from liability for injuries, damages, or claims arising from participation, except where prohibited by law.
Appointments may be rescheduled or canceled according to the current Comfort Flow Recovery cancellation policy. Late cancellations or missed appointments may be subject to fees.
